Assumption of risk
Participation in physical activity carries inherent risk. Clients acknowledge that they participate voluntarily and at their own pace, using the modifications offered.
Health representations
Clients confirm the accuracy of their health and fitness readiness responses and agree to notify BARRE4U of any change in health status, injury, pregnancy, or physician restriction.
Not medical care
Instructors provide fitness instruction only. Completing the screening or signing this waiver does not constitute medical clearance to exercise.
